Can You Trust Gut Microbiome Test Results? Research Suggests Caution
In an era where personalized health and wellness are paramount, direct-to-consumer gut microbiome testing has surged in popularity. These kits promise to unlock insights into the complex ecosystem within our digestive tracts, often suggesting dietary changes or probiotic supplements to optimize health. However, a groundbreaking study has cast a significant shadow of doubt over the accuracy and consistency of these widely marketed tests, urging consumers to exercise caution before making critical health decisions based on their results.
The National Institute of Standards and Technology (NIST), in collaboration with researchers from the University of Maryland, developed a standardized fecal sample to rigorously assess the performance of various microbiome testing companies. This carefully prepared sample, created by homogenizing bowel movements from multiple individuals, served as a biological benchmark. The goal was to determine how reliably different companies could identify the microbial composition within an identical sample.

Seven direct-to-consumer microbiome testing companies were each provided with three identical aliquots of this standardized fecal sample. The findings, published in the journal Communications Biology, revealed a startling lack of consistency. The results returned by these companies varied dramatically, mirroring the discrepancies often observed when analyzing samples from entirely different individuals. This suggests that the microbial profiles reported may not accurately reflect a person's unique gut environment.
Stephanie Servetas, a microbiologist at NIST, explained the purpose of the standardized sample during a news briefing. "We know that it is identical in biology across all those samples," she stated. "What this material is meant to do is really to say how reproducible are the results, either between companies or within a company, but it’s not going to be able to tell us who was closest to the correct answer." While this indicates the test's ability to assess reproducibility, the wide variation itself is a major concern.
The study highlighted specific instances of significant divergence. While some companies maintained a degree of consistency across the three identical samples they analyzed, one company produced a drastically different result for one of the samples. Alarmingly, this company classified two of the results as 'healthy' and the outlier as 'unhealthy,' a classification that could have serious implications for a consumer's health perception and subsequent actions.
Other companies identified many of the same bacterial types but reported significantly different proportions of these microbes. These variations in reported quantities are crucial, as the balance and abundance of specific bacteria are often key indicators of gut health. The level of inconsistency observed on the standardized sample was comparable to the natural variability found between samples collected from different people, underscoring the potential unreliability of the tests.
The implications of these findings are profound. Consumers might be influenced to take unnecessary or even harmful probiotics, adopt restrictive or unbalanced diets, or consider drastic interventions like fecal transplants based on inaccurate microbiome data. Researchers emphasized that such decisions, made on faulty information, could potentially lead to adverse health outcomes.
NIST began distributing its standardized fecal sample to companies last year, aiming to encourage calibration and quality control measures. Servetas expressed hope that this initiative would eventually lead to improved testing methodologies. She clarified that the intention is not to stifle innovation but to establish a baseline for consistency. "There should be some minimum guidelines and some controls" to ensure more reliable results, she stated.
